HOME HEALTH VISIT NOTE (PT) Client: Maria (M.R.) Date: 2026-09-23 HOMEBOUND STATUS Requires rolling walker and assistance of one to leave the home. Leaving home is a taxing effort. SUBJECTIVE Client reports feeling stronger this week and less short of breath on the stairs. Daughter present for the visit. OBJECTIVE MEASURES - Gait distance with rolling walker: 150 feet (supervision) - Sit to stand from kitchen chair: 5 repetitions, no upper extremity support SKILLED INTERVENTIONS - Gait training with rolling walker on carpet and vinyl, cues for step length - Sit to stand progression, hands free - Reviewed home exercise program with daughter SKILLED SERVICE JUSTIFICATION Skilled PT required to progress gait and transfer training safely and to train caregiver in guarding. RESPONSE TO TREATMENT Tolerated well. No adverse response. Rested once during gait. HOME EXERCISE PROGRAM Sit to stand 2 sets of 5, twice daily. Marching in place holding the counter, 1 minute. EDUCATION / CAREGIVER TRAINING Daughter instructed in contact guard for stairs. Return demonstration correct. GOAL PROGRESS - Ambulate 200 feet with rolling walker, supervision: Progressing - Independent sit to stand from standard chair: Progressing ASSESSMENT Progressing toward goals. Continue plan of care. NEXT VISIT PLAN Continue gait training, add outdoor surfaces. Begin stair training with caregiver.
