School service logs that hold up to a Medicaid review
Goal linkage, educational relevance, and the wording that matters.
A school service log is a billing document as much as a clinical one. The reviewer needs four things on every log: the IEP goal or objective addressed, the service delivered and its setting, the student's response, and educational relevance.
Link the goal
Quote or reference the goal as written in the IEP. "Goal 2: produce /r/ in words with 80% accuracy" is verifiable. "Articulation" is not.
Individual, group, or consultation
Say which, and where. Group sessions need the same goal linkage for each student.
Educational relevance
One sentence tying the skill to the school day: participation, reading aloud, navigating the building, using classroom tools.
Progress reports
